Account Authorization Form

Students not in the College of Science and Engineering must fill out the following form and get it authorized by a faculty member in the College, explaining why they need a CSE Labs account.

You must take this completed form to Keller Hall room 1-213.

College of Science and Engineering Systems Staff reserves the right to contact the
professor (or other appropriate person) for verification
and to deny the account whenever appropriate.

Please print legibly; if we can't read it, we cannot grant you authorization for your account.
